Do I Need a Referral for Physiotherapy in Alberta?

No. In Alberta, physiotherapists are primary-contact health professionals, so you do not need a doctor’s referral to book an assessment or treatment at a community clinic. You can call Next Step Physio & Neurorehab Clinic in Edmonton and be seen directly. The exception is insurance: some extended health plans and insurers may ask for a doctor’s note before reimbursing.

This page explains what direct access means, when a referral may still be useful, and how we work with your doctor when needed. Our Millwoods clinic accepts self-referred patients.

What direct access means for you

Physiotherapists in Alberta are regulated by the College of Physiotherapists of Alberta and are trained to assess and treat injuries and conditions without a physician’s order. Direct access means you can:

  • Book an assessment as soon as a problem starts, without waiting for a doctor’s appointment
  • Be assessed, diagnosed within the physiotherapy scope, and start treatment at the same visit
  • Be referred onward by your physiotherapist if something needs medical attention or imaging

Seeing a physiotherapist early is often helpful for sprains, strains, back and neck pain, sports injuries and post-surgical rehabilitation. Your physiotherapist will screen for signs that need a doctor’s input and will tell you if they find any.

When a referral or doctor’s note may still be needed

Although the clinic does not need one, a referral or note can matter in these situations:

  • Extended health insurance: some plans require a doctor’s referral before reimbursing physiotherapy. Check your plan booklet or ask your insurer. If needed, the note can usually be dated after your first visit, but confirm with your plan.
  • WCB-Alberta: a work injury needs a reported claim. You may see a physiotherapist early, but the claim and a doctor’s report support approval of ongoing treatment.
  • Motor vehicle accidents: treatment under Alberta’s Diagnostic and Treatment Protocols involves a primary health care practitioner assessment, which a physiotherapist can provide. Your insurer may also request medical documentation.
  • Publicly funded physiotherapy: hospital and community programs funded by Alberta Health Services have their own eligibility and usually require a physician referral.

If you are unsure, call reception before booking. We can tell you what your insurer is likely to need.

When you should see a doctor first

Direct access does not replace medical care. Please see a physician or go to urgent care first if you have:

  • A suspected fracture or a joint that looks deformed
  • Severe pain following a fall or accident
  • Numbness in the saddle area, loss of bladder or bowel control, or weakness spreading in your legs
  • Fever, unexplained weight loss or night pain that does not ease with rest
  • Chest pain, sudden severe headache, or a new neurological symptom such as facial drooping or slurred speech

In these cases, once you have been assessed medically, physiotherapy is often part of the recovery plan and we can coordinate with your care team.

FREQUENTLY ASKED QUESTIONS

FAQ

Referrals and booking

Can I see a physiotherapist without seeing my doctor?

Yes. Physiotherapists in Alberta are direct-access providers. You can book an assessment at any time.

Will you send a report to my doctor?

With your consent, yes. Many patients ask us to share assessment findings and progress notes with their family physician.

My insurer wants a referral. What should I do?

Ask your doctor for a short note recommending physiotherapy. Many plans accept a note dated after your first visit, but confirm with your insurer.

Can a physiotherapist order X-rays or MRI?

In most community settings, imaging is ordered through your physician. If your physiotherapist thinks imaging is warranted, they will write to your doctor.

Do I need a referral for massage or acupuncture?

No. You can book massage therapy and acupuncture directly. Some plans require a note for reimbursement.

Claims and programs

Do I need a referral for a WCB claim?

You need a reported and accepted WCB-Alberta claim. A doctor's report usually accompanies it, but you can be seen by a physiotherapist early in the process.

Do I need a referral after a car accident?

Not to see us. Under Alberta's Diagnostic and Treatment Protocols, a physiotherapist can complete the initial assessment. Your insurer may still request medical documentation.

Is a referral needed for publicly funded physiotherapy?

Usually yes. Programs funded by Alberta Health Services generally require a physician referral and have eligibility criteria.

What if my doctor already gave me a referral?

Bring it. It is helpful for coordinating care and for insurance, although it is not required to book.

Can I be referred by another physiotherapist or chiropractor?

Yes. We accept referrals from any health professional and also see patients who simply call us.
