Pelvic Pain Physiotherapy in Edmonton
Pelvic pain physiotherapy treats pain in and around the pelvis that often involves the pelvic floor muscles, nerves and joints. At Next Step Physio & Neurorehab Clinic in Edmonton, our pelvic floor physiotherapist assesses muscle tension, movement and breathing, then uses hands-on treatment, relaxation training and graded exercise that may help reduce pain and restore comfort.
We see adults of all genders with pelvic pain that has lasted weeks, months or years, including pain linked to endometriosis, interstitial cystitis, prostatitis-like symptoms, vulvodynia, tailbone injury and pain after surgery or childbirth. No referral is needed, and visits are direct-billed to many extended health plans.
What pelvic pain can look like
Pelvic pain does not always feel like it comes from the pelvic floor. People describe aching in the lower abdomen, groin or hips; burning or sharp pain at the perineum, vulva, penis or rectum; tailbone pain with sitting; pain with intercourse, orgasm or ejaculation; pain with urination or bowel movements without infection; and a constant sense of tightness or pressure.
In many of these cases the pelvic floor muscles are overactive: held tight for long periods, often without the person realizing it. Tight muscles can compress nerves, limit blood flow and become tender, which keeps the pain cycle going. Stress, bracing, breath-holding and prior injury can all feed into this pattern.
How physiotherapy approaches pelvic pain
Treatment for pelvic pain is usually about down-training rather than strengthening. Commonly used approaches include:
- Education about pain and the pelvic floor, so symptoms make sense and feel less threatening
- Breathing and relaxation techniques to release a habitually tight pelvic floor
- Manual therapy to tender muscles, scars and connective tissue in the pelvis, hips and low back
- Gentle stretching and mobility work for the hips, spine and pelvis
- Graded return to sitting, exercise and intimacy at a pace your body accepts
- Bladder and bowel strategies where pain is linked to urgency or constipation
Mercy, our pelvic floor physiotherapist, provides pelvic pain care at Next Step. Where useful, we coordinate with your doctor, gynaecologist, urologist or counsellor so care fits together.

When to see a doctor first
Pelvic pain has many possible causes, and physiotherapy works alongside medical care, not instead of it. Please see your doctor before booking if you have fever, blood in your urine or stool, unexplained weight loss, new or severe pain, pain during pregnancy, or pain that started after trauma. Infections, gynaecological and urological conditions and other medical issues need to be assessed and treated appropriately.
If you have already been assessed and told your tests are normal, or you have a diagnosis and want help managing the muscular and nervous-system side of the pain, physiotherapy is often a reasonable next step.
